Targeted Case Management
Children struggling with diagnosed mental health challenges receive help, hope and guidance through Targeted Case Management. We work closely with families to connect them with the right community resources and counseling to help children thrive in their own homes.
Targeted Case Managers develop and maintain a coordinated plan of care for children who have serious mental health problems that require the support of multiple agencies and multiple mental health services. Targeted Case Managers’ goals are to optimize the child’s development, strengthen the family’s competence and maximize available resources. This service can only be provided to children who are currently receiving counseling services. If a referral is made for counseling, an assessment will also be made at the time of the intake to determine if the child is eligible for targeted case management.
Many families in this program are struggling to make ends meet; as we help them navigate complex systems to assure their children receive necessary services, we ask our community to help the families meet basic needs so they can help their children work through challenges.
